Heart Healthy Ohio Initiative (HHOI) is a statewide cardiovascular health collaborative funded by the Agency for Healthcare Research and Quality that has been working with 50 primary care practices across the state to improve blood pressure and smoking cessation as well as reduce disparities in cardiovascular disease.
